- Document NIHSS
- If not a stroke alert, then statement in ED note: ‘Thrombolytics considered, not given due to ________________’
- Mild non-disabling stroke symptoms
- NIH of 0
- Thrombin or factor Xa inhibitors (novel oral anticoagulants) within 48 hours

- Must document statement of thrombolytic consideration.
- ‘Thrombolytics considered’ does not count.
- ‘Patient symptoms are completely resolved’ is like stating ‘Rapid Improvement’, and neither of those statements are acceptable contraindications.
‘Non-disabling’ is the phrase that must be used.
- DO NOT USE THE PHRASES “Rapid improvement”, “Low NIHSS”, or “Symptoms resolved” unless linked to an accepted contraindication.
